Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to train patients for home dialysis and provide ongoing support. You will perform home visits, evaluate environments, document care, and coordinate with physicians to ensure high-quality outcomes.

The role requires an active RN license, CPR, and home dialysis orientation, with on-call coverage and a focus on infection control and patient safety. A collaborative multi-disciplinary team supports your work.

Qualifications

• Current RN license in applicable state; must be maintained in good standing.
• 12 months of clinical nursing experience; 3 months in home dialysis modality required in most states.
• CPR certification required prior to patient care assignment.
• Ability to distinguish all primary colors and communicate effectively.
• Complete Home Therapy Nurse Orientation and maintain competency for the modality assigned.

Responsibilities

• Provide patient training and ongoing support for home dialysis options.
• Conduct home visits to assess home environment and patient performance; adjust as needed.
• Document nursing services in EMR; reflect patient status and interventions.
• Communicate assessment results to physicians and care team.
• Participate in infection control monitoring and Emergency Preparedness procedures.
• Manage clinic and patient supply inventory and timely orders to avoid penalties.
• Assist in continuous quality improvement activities and data collection.
• Administer medications as ordered and perform monthly medication profile reviews.
• Provide on-call nursing coverage during nights/weekends on a rotational basis.
