Job summary

U.S. Renal Care, Inc. seeks a Home Therapy Registered Nurse to train patients in home dialysis and provide ongoing support, ensuring high-quality care and superior service.

The role requires active RN license, 12 months nursing experience with 3 months in home modality, CPR certification, and ability to work across clinics and home visits. Flexible scheduling and strong communication skills are essential.

Qualifications

• Current RN license in applicable state.
• 12 months of clinical nursing experience with 3 months in the home dialysis modality (or as required by state).
• CPR certification required prior to patient care assignment.
• Ability to distinguish primary colors.
• Proficiency with English language (verbal and written).
• Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.

Responsibilities

• Train patient (and/or care partner) in self-care for home dialysis.
• Conduct home visits to assess patient home environment and care needs.
• Communicate patient assessment results to physician and care team.
• Document nursing services in the EMR ensuring continuity of care.
• Participate in infection control monitoring and reporting.
• Be familiar with emergency equipment and procedures; discuss emergency plans with home patients.
• Manage clinic and patient supply inventory and place orders timely.
• Support continuous quality improvement through data collection and analysis.
• Obtain routine and non-routine laboratory tests as ordered and communicate critical values.
• Administer medications as ordered and perform on-call nursing services as needed.
• Maintain flexible staffing locations and hours to meet patient needs.
