Job summary

Fresenius Medical Care is seeking an entry level Home Therapies RN CAP 1 to coordinate and deliver clinically competent care for a defined patient group in the home dialysis program. You will assess responses to training, develop care plans, educate patients and families, and collaborate with the physician and team to ensure quality outcomes.

The role includes medication administration, documentation, and participation in on-call rotations.

Qualifications

• RN with state licensure and CPR/BLS certification.
• Entry level RN, prev. home dialysis experience preferred.
• Graduate of an accredited School of Nursing.

Responsibilities

• Coordinate care for an assigned patient group.
• Assess and document patient responses to home dialysis training and therapy.
• Educate patients and families on home dialysis processes and safety.
• Administer prescribed medications and document effectiveness.
• Collaborate with the interdisciplinary team for care planning.
• Participate in on-call rotations and continuous quality improvement.
