Job summary

Fresenius Medical Care seeks an entry level Home Therapies RN CAP1 to coordinate and deliver clinically competent care for an assigned group of patients on home dialysis. Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• RN with less than 2 years nephrology nursing experience is entry level for CAP.
• Minimum 12 months RN experience.
• Home dialysis therapy experience preferred.

Responsibilities

• Provide and coordinate clinically competent care for an assigned group of patients.
• Assess, plan, implement and evaluate care for home dialysis patients.
• Educate patients and families on home dialysis training and treatment.
• Document care, monitor patient response, and communicate with physicians as needed.
• Ensure safe delivery of dialysis therapy and participate in quality improvement activities.
