Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to coordinate and deliver clinically competent home dialysis care. The role includes assessment, planning, education for patients and families, and collaboration with the care team.

As part of the End Stage Kidney Disease team, you will contribute to quality improvement, participate in training, and support ongoing CAP advancement requirements.

Qualifications

• Graduate of an accredited School of Nursing and active state licensure.
• Current CPR BLS certification required.
• RN with entry to mid-level nephrology/home dialysis experience preferred.

Responsibilities

• Assess, plan, implement and evaluate care for home dialysis patients.
• Educate patients and families on home dialysis and treatments.
• Collaborate with interdisciplinary team to develop and update care plans.
• Lead and mentor staff and participate in training activities.
• Ensure proper lab collection, documentation and protocol adherence.
