Job summary

Fresenius Medical Care seeks an entry-level Home Therapies RN CAP 1 to join the Clinical Advancement Program. You will provide and coordinate clinically competent care, assess and educate patients on home dialysis, and collaborate with the care team to improve outcomes.

You will participate in CAP requirements, support the Interdisciplinary Team, and assist with lab procedures while ensuring safety and quality in home dialysis delivery.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure in good standing.
• CPR/BLS certification required.
• Must meet the practice requirements in the state of employment.

Responsibilities

• Provide clinically competent care with assessment, planning, intervention and evaluation for an assigned group of patients.
• Coordinate dialysis data collection and adjust treatment per protocol; notify physician as needed.
• Educate patients and families on home dialysis training and treatment options.
• Lead and train staff, participate in staff development and quality improvement activities.
• Ensure proper lab collection, processing, and documentation in the medical record.
