Job summary

Fresenius Medical Care seeks an entry-level Home Therapies RN CAP 1 to join the Clinical Advancement Program. You will assess and coordinate care, train patients and families in home dialysis, and collaborate with the health care team to improve patient outcomes.

Responsibilities include administering medications, updating care plans with the interdisciplinary team, and participating in on-call rotations and education of staff. BSN is preferred and state licensure is required.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current CPR/BLS Certification.
• Must meet practice requirements in the state of employment.

Responsibilities

• Provide and coordinate clinically competent care for an assigned patient group.
• Assess, plan, implement and evaluate home dialysis training and treatment therapy.
• Educate patients and families about care plans and home dialysis.
• Administer medications per protocol and document effectiveness.
• Participate in on-call rotations and emergency response as needed.
