Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to provide and coordinate clinically competent care for an assigned group of patients, focusing on home dialysis training and treatment outcomes. You will assess, plan, intervene, and evaluate patient care with guidance from supervisors and educators.

Responsibilities include education for patients and families, collaboration with the Interdisciplinary Team, and participation in ongoing quality improvement activities.

Qualifications

• Entry level RN with less than 2 years of nephrology nursing experience.
• Home dialysis therapy experience is preferred.

Responsibilities

• Provide and coordinate home dialysis care per Fresenius Kidney Care protocols.
• Assess patient learning needs and educate patients and family.
• Administer medications as prescribed and document outcomes.
• Collaborate with Interdisciplinary Team to update care plans.
• Train Home Dialysis patients and Home Partners on equipment and procedures.
• Participate in on-call rotation and possible travel as needed.
