Job summary

Fresenius Medical Care in Oregon seeks an entry level Home Therapies RN CAP 1 to join the Clinical Advancement Program. This role coordinates and delivers clinically competent care, including assessment, planning, intervention and evaluation for an assigned group of patients undergoing home dialysis training and treatment therapy.

You will communicate patient issues to physicians as needed and participate in teaching, leadership and quality improvement activities to enhance patient care outcomes

Qualifications

• Entry level RN with nephrology nursing experience.
• Home dialysis experience preferred.
• Must meet nursing practice requirements in the state employed.

Responsibilities

• Performs all essential functions under supervision with guidance from Educator or Preceptor.
• Collects and analyzes dialysis data for assigned patients and documents in medical record.
• Assesses and educates patient/family on learning needs and home training.
• Trains patients and Home Partners on safe operation of home dialysis equipment.
• Directs safe and effective delivery of dialysis therapy with the care team.
• Performs medication administration as prescribed or per approved algorithms.
• Participates in emergency response measures and staff training.
• Ensures proper lab collection, processing, and shipping; reschedules missed collections.
• Updates nursing assessment and plan of care in collaboration with Interdisciplinary Team.
• Explains transplant and treatment modality options to patients.
