Job summary

Fresenius Medical Care in Newburyport, MA is seeking an entry-level Home Therapies RN CAP to begin the Clinical Advancement Program. You will assess, plan, and evaluate care for an assigned patient group, coordinate home dialysis education, and collaborate with physicians and the care team to optimize patient outcomes.

Responsibilities include data collection, treatment adjustments, medication administration, and participation in training and quality initiatives.

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure.
• Current or successful CPR BLS Certification.

Responsibilities

• Provides clinically competent care including assessment, planning, intervention and evaluation.
• Collects and analyzes dialysis data and adjusts treatment plans with physician notification as needed.
• Educates patients and families on home dialysis procedures and safety.
• Directs and provides safe delivery of dialysis therapy with home care partners and staff.
• Administers medications as prescribed or via approved algorithms.
• Participates in emergency response measures and staff training.
• Ensures proper lab collection, processing and shipping and reschedules missed labs.
• Documents nursing assessments and plans of care with the interdisciplinary team.
