Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for Home Therapies under the CAP program. You will provide and coordinate clinically competent care for an assigned group of patients, including assessment, planning, intervention and evaluation of home dialysis training and treatment therapy.

You will work with the End Stage Kidney Disease care team, participate in decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility

Qualifications

• Graduate of an accredited School of Nursing.
• Current valid RN license in the state of employment.
• CPR BLS certification valid at date of hire.
• Must meet state practice requirements where employed.

Responsibilities

• Provide and coordinate clinically competent care for home dialysis patients.
• Assess and manage patient response to home dialysis training and treatment.
• Educate patients and families about home dialysis protocols and plans.
• Administer medications per approved algorithms and document effectiveness.
• Collaborate with interdisciplinary team to update care plans.
• Assist in on-call rotation and travel to training sites or patient homes.
