Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN for the Clinical Advancement Program (CAP). You will assess, plan, implement and evaluate care for an assigned group of home dialysis patients, following protocols and collaborating with physicians.

Education includes nursing licensure and CPR/BLS; you will educate patients and families, collect dialysis data, and support the home dialysis team. This role may involve on-site clinic duties and patient homes as needed.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification
• Must meet the practice requirements in the state in which he or she is employed.

Responsibilities

• Performs essential functions under supervision with guidance from educators or other RNs.
• Collects and analyzes dialysis data, documents in medical records, and notifies physicians as needed.
• Assesses and educates patients and families on learning needs and care plans.
• Directs safe delivery of dialysis therapy with the home care team.
• Administers medications as prescribed and documents effectiveness.
• Participates in emergency response and staff training.
