Job summary

Fresenius Medical Care is seeking an entry-level Home Therapies RN CAP 1 to join the Clinical Advancement Program. The role includes coordinating care, assessing patient response to home dialysis training, and communicating with physicians as needed.

Responsibilities include education, data collection, and collaboration with the interdisciplinary team to improve patient outcomes and program operations.

Qualifications

• RN with entry-level CAP nursing track in dialysis
• Must be a graduate of an accredited School of Nursing and hold active state licensure
• CPR BLS certification required or current
• Home Dialysis/Nephrology experience preferred

Responsibilities

• Provide and coordinate clinically competent home dialysis care
• Assess, plan, implement, and evaluate patient care for assigned patients
• Educate patients and families about home dialysis and transplant options
• Document dialysis data and adjust care plans as needed
• Train Home Dialysis patients and Home Partners in equipment operation and safety
