• Communicate schedules with clients and staff.
• Occasionally conduct in-home client visits to maintain connections.
• Prepare routine departmental reports and document client status.
• Record client condition and services accurately.
• Handle client complaints and report significant issues.
• Provide education and coaching on Plan of Care changes.
• Maintain positive relationships with clients and staff.
• Ensure compliance with laws and company policies.
• Perform other related duties as assigned.
