Job summary

Help at Home is seeking a Client Service Supervisor to oversee care coordination and ensure high-quality client services in Indiana. You will manage client files, schedules, and communications with clients, aides, and referral sources, while upholding HIPAA guidelines.

The role involves coaching caregivers, conducting in-home visits when needed, and maintaining accurate documentation and client satisfaction records to support a compliant, client-centered care program.

Qualifications

• High school diploma or equivalent required; 18+ year age requirement.
• PPD or Chest x-ray if applicable.
• Medicaid, Waiver, or Home Healthcare experience preferred.

Responsibilities

• Maintain client files and related paperwork.
• Maintain confidentiality of patient/client and agency information per HIPAA guidelines.
• Assigns homecare aides based on client needs and Plan of Care.
• Coach Caregivers to ensure high-quality client care; escalate concerns.
• Communicate schedules and changes with clients, aides, referrals, and management.
• Conduct in-home visits to assess needs and communicate changes.
• Prepare and submit routine departmental reports.
• Document client condition and overall service accurately.
• Monitor client satisfaction surveys and in-home visits; address concerns.
• Handle client complaints and issues; document for resolution; mandatory reporter.
• Educate and coach on Plan of Care changes.
• Maintain positive relationships with clients, aides, and referrals.
• Ensure compliance with laws and company policies.
