Responsibilities

• Maintain client files and related paperwork.
• Protect confidentiality per HIPAA guidelines.
• Assign homecare aides based on Plan of Care.
• Coach caregivers to ensure high-quality care and escalate concerns.
• Communicate schedules to clients, aides, referrals, and management.
• Prioritize client care and may perform in-home visits.
• Prepare and submit routine departmental reports.
• Document client condition and service records.
• Record client satisfaction surveys and visits.
• Handle client complaints and escalate issues as required.
• Provide education on changes to Plan of Care.
• Maintain relationships with clients, aides, referrals sources.
• Ensure compliance with laws and company policies.
• Perform other duties; may involve state regulations for DD focus.
