Job summary

Soap Creek Resources is seeking a results-driven Business Development Specialist to expand community partnerships and increase patient access to home care services in NYC, Long Island, Westchester, and nearby markets. You will build referral networks and educate partners on services, collaborating with intake to ensure smooth starts of care.

Ideal candidates bring experience in home care outreach or MLTC programs, a proven track record of generating referrals, and strong communication skills.

Qualifications

• Experience in home care marketing or healthcare business development.
• Proven ability to generate referrals and starts of care.
• Strong communication and relationship-building skills.
• Valid driver’s license and willingness to travel locally.

Responsibilities

• Build and maintain relationships with physicians, hospitals, rehabilitation centers, community organizations, and other referral sources.
• Identify and develop new referral opportunities throughout the assigned territory.
• Represent the agency at community events, healthcare offices, and outreach initiatives.
• Educate referral partners on available home care services.
• Collaborate with the intake team to ensure a smooth transition from referral to patient enrollment.
• Drive consistent referral activity that converts into new patient starts of care, meeting monthly growth goals.
• Manage your territory independently while maintaining regular outreach and follow-up activities.
