Ideal candidates have experience in home care outreach, Medicaid/MLTC programs, and proven ability to generate referrals and enrollments.

Qualifications

• Experience in home care marketing or healthcare outreach.
• Proven ability to generate referrals and patient enrollments.
• Strong communication, presentation, and relationship-building skills.

Responsibilities

• Build and maintain relationships with physicians, hospitals, and community organizations.
• Identify and develop new referral opportunities within the territory.
• Represent the agency at community events and healthcare offices.
• Educate partners on available home care services.
• Collaborate with intake to ensure smooth enrollment from referral to patient starts of care.
• Drive referral activity to meet monthly growth goals.
