Job summary

CareerNerds, LLC is hiring a Business Development Specialist to expand our referral network and grow patients in NYC, Long Island, and Westchester. The role emphasizes relationship-building with physicians, hospitals, and community organizations to increase starts of care for home health services.

The ideal candidate has experience in home care outreach or Medicaid/MLTC programs, strong communication skills, and the ability to travel locally within the tri-state area.

Qualifications

• Experience in home care marketing or healthcare outreach.
• Proven ability to generate referrals or starts of care.
• Strong communication and presentation skills.
• Ability to work independently in a defined territory.

Responsibilities

• Build and maintain relationships with physicians, hospitals, rehab centers, community organizations, and other referral sources.
• Identify and develop new referral opportunities in the assigned territory.
• Represent the agency at community events and outreach initiatives.
• Educate referral partners on available home care services.
• Collaborate with the intake team to ensure smooth transition from referral to enrollment.
• Drive referral activity to achieve monthly growth goals.
• Manage the territory independently with regular outreach and follow-up.
