Job summary

Lehigh Valley Health Network is seeking a dedicated Occupational Therapist for Home Care. The role involves evaluating, planning, and guiding OT services to restore function and promote independence in patients receiving home care.

You will design individualized plans, utilize evidence-based techniques, and coordinate with families and care teams to support daily living activities. Ideal candidates hold a Bachelor’s in OT with PA licensure and BLS, and have the ability to travel to patient care

Qualifications

• Bachelor’s Degree in Occupational Therapy required.
• Licensed Occupational Therapist in the State of Pennsylvania required.
• Eligible to travel to patient care assignments (Home Care).
• Basic Life Support (BLS) certification required.

Responsibilities

• Establishes and implements occupational therapy plans of care to restore function and prevent disability.
• Implements individualized treatment plans using evidence-based techniques.
• Provides OT treatment aligned with patient needs assessment and goals.
• Performs reassessment and updates plans as needed.
• Communicates patient progress to supervisor and team members.
• Ensures documentation complies with policies and regulatory requirements.
