Advanced Home Health and Hospice of Lehigh Valley is seeking a dedicated Occupational Therapist to provide home-based OT services, helping patients regain independence in daily activities within their own homes.

This PRN role covers weekdays daytime visits in the Lehigh Valley area; requires PA OT license, driver’s license and reliable transportation; prior home health or hospice experience is preferred; strong assessment, communication, and documentation skills are essential.

Qualifications

• Current, active Occupational Therapist license in Pennsylvania (required).
• Previous home health or hospice experience preferred.
• Strong clinical assessment and problem-solving skills.
• Compassionate, patient-centered approach.
• Excellent communication, organization, and documentation abilities.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Perform comprehensive occupational therapy evaluations and develop individualized care plans.
• Deliver skilled occupational therapy interventions to home health and hospice patients.
• Instruct patients and caregivers on therapeutic exercises, adaptive equipment, safety, and techniques to improve independence in ADLs.
• Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated care.
• Monitor patient progress, adjust treatment plans as needed, and document all care provided.
• Educate patients and families on home safety modifications and techniques to prevent falls and injuries.
• Travel to patient homes within the Lehigh Valley and surrounding areas.
