Job summary

Lehigh Valley Health Network (LVHN) invites you to join our Home Care - Occupational Therapy team as a full-time OT. In this role, you will evaluate patients, design individualized plans of care, and support independence through therapeutic activities and education in patients' homes.

The position emphasizes evidence-based practice, collaboration with families, and safe, compassionate care. Travel to patient sites is required; PA licensure and BLS are needed upon hire.

Qualifications

• Bachelor’s Degree in Occupational Therapy.
• Less than 1 year clinical experience required.
• Licensed OT in Pennsylvania upon hire.
• Ability to travel to patient care assignments.

Responsibilities

• Establishes and implements OT plans of care to restore function and independence.
• Implements therapy techniques: exercises, manual therapy, ADL training, splinting, DME.
• Provides patient education and home safety guidance.
• Communicates progress and limitations to supervisor and care team.
• Ensures documentation meets policies and regulatory requirements.
