Qualifications

• High school graduate or equivalent preferred; may require higher education or certification.
• Experience with Medicaid, Waiver, or Home Healthcare preferred.

Responsibilities

• Maintain client files and related paperwork.
• Maintain confidentiality of client and agency information per HIPAA guidelines.
• Assign homecare aides based on client needs and plan of care.
• Coach caregivers to ensure high quality client care and escalate issues.
• Communicate schedules and changes with clients, aides, referral sources and management.
• Possibly conduct in-home visits to assess needs and communicate changes.
