Job summary

Help at Home is seeking a Client Service Supervisor to support clients with intellectual and developmental disabilities. You will help develop and implement a Person-Centered Individualized Support Plan (PCISP) and coordinate caregiver schedules with the team.

Office hours are weekday mornings, with varying hours to meet client needs. This role emphasizes relationship-building, training delivery, and thorough documentation in EMR systems.

Qualifications

• Strong verbal and written communication skills.
• Organizational skills with focus on record-keeping and prioritization.
• Independent work ethic and problem-solving ability.
• Experience with clients who have intellectual or developmental disabilities is a plus.
• A insured personal vehicle and a valid driver’s license required.
• Clean background checks and pass a drug test.

Responsibilities

• Build and maintain relationships with clients, family members, caregivers, case managers, and team members through regular check-ins and follow-through.
• Coordinate caregiver schedules and PCISP-based training.
• Document client schedules, PCISP budgets, and quarterly/annual reports.
• Attend quarterly meetings to review PCISP and support strategies.
• Keep accurate records in EMR and other systems.
