Qualifications

• Age requirement and education documented per state rules.
• Clearance checks (PPD or Chest X-ray) as applicable.
• Experience in Medicaid, Waiver, or Home Healthcare preferred.
• Demonstrated compassion and professional communication.

Responsibilities

• Maintain client files and paperwork.
• Maintain confidentiality per HIPAA guidelines.
• Assigns homecare aides based on care plan.
• Coach caregivers to ensure high-quality care and escalate concerns.
• Communicate schedules with clients, aides, referrals and management.
• Conduct in-home visits to assess needs and changes.
• Prepare and submit routine departmental reports.
• Document client condition and service delivery.
• Monitor client satisfaction and address concerns.
• Report significant issues as mandatory.
• Educate clients on changes to Care Plans.
• Maintain positive relationships with clients and staff.
• Ensure compliance with laws and company policies.
