Help At Home, LLC seeks a Client Service Supervisor to lead client care coordination and caregiver performance in Delaware. You will manage client files, schedules, and Plan of Care changes while ensuring confidentiality and regulatory compliance.

Responsibilities include coaching caregivers, communicating with clients and referrals, and conducting in-home visits when needed. The role requires driving quality service and maintaining positive client relationships.

Responsibilities

• Maintain elements of the assigned clients’ files, and all related paperwork.
• Confidentiality per HIPAA guidelines in handling patient records.
• Assigns homecare aides based on client needs and Plan of Care.
• Coaches caregivers to ensure high-quality client care and escalates concerns.
• Communicates schedules and changes with clients, aides, referrals, and management.
• May conduct in-home visits to assess needs and communicate changes.
• Prepares and submits routine departmental reports.
• Records and maintains accurate documentation of client’s condition and service.
• Maintain client satisfaction surveys and in-home visits to address concerns and relationships.
• Handles client complaints; documents and reports significant issues; mandatory reporter for incidents.
• Provides education on changes to Plan of Care.
• Maintains positive relationships with clients, aides, and referral sources.
• Ensures compliance with laws and company policies.
• Performs other related duties as assigned.
• If DDfocus, complies with state regulations for training and records.
