Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient care in the home setting across Leflore, Bolivar, and Sunflower counties. You will perform comprehensive assessments, implement and adjust plans of care, and collaborate with an interdisciplinary team to deliver compassionate hospice services.

The role emphasizes education for patients and families, accurate documentation, and adherence to regulatory standards to ensure high-quality end-of-life care.

Qualifications

• 1+ year RN experience in hospice, home health, oncology, or related setting.
• Current RN license for state of employment.
• CPR certification.
• Strong patient assessment and documentation skills.
• Knowledge of hospice philosophy and end-of-life care.

Responsabilités

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments and education for patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visit frequency based on patient needs and risk.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Educate patients and caregivers on disease process, self-care, end-of-life care, medications, nutrition and dietary needs.
• Document timely clinical records and ensure infection control and regulatory compliance.
