Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to lead in-home hospice care as an RN Case Manager. You will assess patients, implement care plans, and coordinate with an interdisciplinary team to support patients and families in the comfort of their homes.

Responsibilities include ongoing assessments, symptom management, caregiver education, and timely documentation while maintaining regulatory compliance and compassionate communication.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum 1 year experience as an RN in acute care, hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing care per Plan of Care in patients' homes or other settings.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients, families and caregivers about disease, treatment and end-of-life care.
• Document clinical findings, updates and plans accurately and timely.
• Maintain infection control and comply with regulatory standards.
• Participate in on-call shifts and care transitions as needed.
