Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient-focused care at home or in alternative settings. You will perform comprehensive assessments, develop and update plans of care, and coordinate with IDG members to ensure timely interventions.

The role emphasizes education for patients and families, symptom management, bereavement support, and accurate documentation. You will collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to deliver

Qualifications

• RN license in good standing.
• Minimum 1 year RN experience in acute care, hospice, or home health.
• Hospice/palliative care experience preferred.
• Full and active licensure to practice nursing in state of employment.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across IDG and with physicians and other clinicians.
• Educate patients and caregivers about disease, self-care, medications, nutrition, and end-of-life needs.
• Communicate changes in condition to the care team for timely interventions.
• Maintain accurate clinical documentation and ensure regulatory compliance.
• Support on-call and patient support activities as applicable.
