Gentiva Hospice seeks an experienced RN Case Manager to lead in-home hospice care across Leflore, Bolivar, and Sunflower, MS. You will guide patients and families through the hospice journey with compassion as the primary nurse.

You will perform assessments, develop plans of care, educate caregivers, coordinate with an interdisciplinary team, and ensure accurate documentation and compliance. A current RN license and hospice experience are required.

Formación

• Current RN license in the state of employment.
• Minimum of one (1) year RN experience in acute care, oncology, hospice or home health.
• Hospice or palliative care experience preferred.

Responsabilidades

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized care plans including visit frequency to meet needs and risk.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Educate patients and caregivers on disease process, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and communicate changes in condition to the care team.
• Participate in on-call and patient support activities as applicable.
