Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate home-based nursing care across Leflore, Bolivar, and Sunflower regions. You will assess patients, implement plans of care, and collaborate with an interdisciplinary team to ensure comfort and dignity at end of life.

The role includes educating families, coordinating care, and maintaining precise medical records, while upholding compliance and quality standards in a fast-paced home health setting.

Qualifications

• Current license to practice nursing in the state of Mississippi (or applicable state).
• Hospice/palliative care experience preferred.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit scheduling.
• Coordinate care with physicians and IDG members.
• Educate patients and families on disease, end-of-life care, medications, and care techniques.
• Document all care accurately and timely.
