Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead comprehensive nursing care in patients' homes, assisted living facilities, or other residences. You will assess needs, develop and adjust plans of care, and collaborate with an interdisciplinary team to ensure high-quality, compassionate end-of-life care.

The role emphasizes education for families, symptom management, and coordination across physicians, social workers, chaplains, and other care providers.

Qualifications

• Registered Nurse with active Florida license.
• Minimum 1 year RN experience in hospice, home health, or related field.
• Florida Level 2 background screening required.
• CPR certification.

Responsibilities

• Be the primary point of contact for patients and families throughout the hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home, assisted living, or other home settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDG to refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
