Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in home-based hospice care. You will perform comprehensive assessments, develop plans of care, and coordinate with an interdisciplinary team to deliver compassionate, patient-centered services.

You will educate patients and families, document care accurately, and ensure regulatory compliance, infection control, and quality improvement initiatives.

Qualifications

• RN licensure in the state of employment
• Minimum of 1 year RN experience in acute care, oncology, long-term care, hospice, or home health
• Hospice or palliative care experience preferred
• Current CPR certification
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments and education for patients, family members and caregivers
• Deliver skilled nursing interventions and symptom management aligned with goals of care
• Develop and update individualized plans of care including visit frequency to meet patient needs
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners
• Provide patient and caregiver education regarding disease process, self-care, end-of-life care, medications, nutrition and dietary needs
• Communicate changes in condition or signs of decline to appropriate providers
• Support continuity of care by recognizing referrals and transitions in levels of care
• Document timely clinical records on all patients including orders, progress notes and care plans
• Maintain effective working relationships with the IDG and involve team as needed
• Participate in on-call and patient support activities as applicable
