Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patients and families through the hospice journey. You will deliver skilled nursing care in patients’ homes or equivalent settings, perform ongoing assessments, and collaborate with an interdisciplinary team to tailor personalized care plans.

The role emphasizes educating caregivers, documenting care, and ensuring timely interventions while supporting emotional and spiritual needs as part of comprehensive end-of-life care.

Qualifikationen

• RN license in the state of employment.
• Minimum of one year experience as an RN, with hospice or palliative care preferred.
• Strong ability to educate patients and families and collaborate with IDG.

Aufgaben

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on the Plan of Care, in home or other settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers, and chaplains to refine care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Teach caregivers practical and emotional care techniques.
• Communicate patient condition updates to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent care.
• Educate patients and families on medications, hospice philosophy, symptoms, and end-of-life processes.
