Gentiva Hospice in Mississippi is seeking a compassionate Hospice RN / RN Case Manager to provide in-home and community-based care. You will assess patients, develop plans of care, and coordinate with an interdisciplinary team to deliver comfort and dignity.

The role requires an active RN license and at least 1 year of hospice or home health experience, strong communication, documentation, and family education. Travel covers Leflore, Bolivar, and Sunflower counties with on-call as needed.

Qualifications

• RN license active and in good standing.
• Minimum 1 year experience as RN in hospice, home health, or related setting.
• Hospice or palliative care experience preferred.

Responsabilités

• Perform comprehensive patient assessments and ongoing evaluations.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across an interdisciplinary team and with physicians.
• Educate patients and families about disease, care options, and end-of-life planning.
• Maintain accurate clinical documentation and ensure regulatory compliance.
