Job summary

Health Connect America seeks a Family Development Specialist to support clients and families in developing and implementing treatment plans within home and community settings. You will deliver individual, group, and family therapies, coordinate services, recruit referrals, and maintain compliant documentation while collaborating with a Clinical Supervisor.

Qualifications

• Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, and skill-building per the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring their needs are met in a comprehensive manner.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities as assigned, to stay current with best practices for quality service delivery.
