Job summary

Health Connect America seeks a Family Development Specialist to support clients and families by developing and implementing treatment plans and linking them to services. This role is based in clients' homes and involves coordinating care with the treatment team and supervisor.

You will provide individual, group, and family interventions, attend weekly Treatment Team meetings, recruit referrals, and maintain accurate documentation while ensuring compliance with COA standards and state

Qualifications

• Requires a Bachelor's degree in a human services field such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field.
• Minimum of two years' experience working with children and/or families (internships/volunteer work count).

Responsibilities

• Develop individualized treatment plans and coordinate care with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building per the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients to services and resources identified in the treatment plan.
• Serve as liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions, treatment plans, and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
