Job summary

Health Connect America, Inc. is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services.

This is a community-based role working in clients' homes. You will recruit new referrals, participate in weekly treatment team meetings, and maintain compliant documentation while ensuring regulatory standards are met.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families, including internships or volunteer work.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the plan.
• Attend weekly Treatment Team meetings and participate actively.
• Link clients to services and resources identified in the plan and act as liaison with professionals and community resources.
• Maintain accurate, timely documentation of client interactions and progress in compliance with standards.
• Recruit new referrals to maintain a full caseload and provide 24/7 on-call support when required.
• Ensure compliance with state regulatory bodies and COA standards; participate in ongoing training.
