Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families in their homes by developing treatment plans, delivering therapeutic interventions, and linking families to needed services. You will collaborate with the treatment team, maintain thorough documentation, and actively recruit new referrals while ensuring regulatory compliance.

The role is community-based, requiring engagement with clients in their homes and coordination with a multidisciplinary team.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as liaison with other professionals, agencies and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans and progress notes in compliance with policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development to stay current with best practices.
