Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans and delivering therapeutic interventions in home and community settings.

You will coordinate care with a treatment team, recruit new referrals, and ensure detailed documentation while adhering to COA standards and state regulations. A Bachelor's degree in a human services field and 2 years of relevant experience are required.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (including internships/volunteer work) is required.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients/families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families to services/resources identified in the plan.
• Serve as liaison with professionals/agencies to meet client needs.
• Maintain accurate documentation of client interactions and progress notes per policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulations and COA standards.
• Participate in ongoing training to stay current with best practices.
