Job summary

Health Connect America seeks a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to services in home and community settings.

You will maintain documentation, participate in weekly Treatment Team meetings, recruit referrals, provide 24/7 on-call support, and ensure regulatory compliance with COA standards in a community-based program serving children, families, and adults.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (including internships and volunteer work).

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, as well as skill-building, outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources as identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources related to the client, ensuring needs are met comprehensively.
• Maintain accurate and timely documentation of all client interactions, treatment plans, and progress notes in compliance with organizational policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with all state regulatory bodies and COA standards.
• Participate in ongoing training and professional development activities to stay current with best practices for quality service delivery.

Education

Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field
