Job summary

Health Connect America, Inc. seeks a Family Development Specialist to support clients and families by developing treatment plans, providing therapeutic interventions, and linking clients to services. This is a community-based role working in clients' homes and communities.

The role requires at least a bachelor’s in a human services field and two years’ experience with children or families, with on-call duties and a full benefits package included.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team.
• Provide individual, group, and family therapeutic interventions per the treatment plan.
• Attend weekly Treatment Team meetings and actively participate.
• Link clients to services and resources per the treatment plan.
• Maintain accurate documentation of client interactions and progress notes per policy.
