Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by creating and implementing treatment plans, delivering therapeutic interventions, and coordinating related services within our multi-state network.

You will work with a treatment team, participate in weekly meetings, and ensure documentation meets regulatory standards while actively recruiting referrals to maintain a full caseload.

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships/volunteer work acceptable).

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients to services and resources identified in the treatment plan.
• Maintain accurate documentation of client interactions, plans, and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
