Health Connect America is hiring a Family Development Specialist to support clients in homes, communities, and offices. You will develop treatment plans, deliver individual, group, and family therapy, and coordinate connected services with the treatment team.

Qualifikationen

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Aufgaben

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services/resources identified in the treatment plan.
• Serve as a liaison with professionals, agencies and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in line with policy/regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development.
