Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services. This is a community-based role working in clients' homes, with a focus on collaborative care and ongoing documentation.

Qualified candidates will have a Bachelor's degree in a human services field and at least two years of experience with children and/or families.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships and volunteer work acceptable).

Responsibilities

• Develop individualized treatment plans and coordinate care for clients with input from the treatment team and the Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Serve as a liaison with professionals, agencies, and community resources to meet clients' needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes in compliance with policy and standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
