Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans and linking them to essential services. This community-based role works within clients' homes and local communities, with a focus on coordinated care and proactive outreach.

Candidates should hold a Bachelor's degree in a human services field and have at least two years of relevant experience working with children and/or families.

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building as outlined in the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of all client interactions, treatment plans, and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
