Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, delivering therapeutic interventions, and connecting clients with essential services in their homes, communities, or offices. The role requires a bachelor’s degree in a human services field and a minimum of two years of experience with children and families.

You will work with a treatment team, recruit referrals, and ensure compliance with COA

Qualifications

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families; internships/volunteer work count.

Responsibilities

• Develop individualized treatment plans and coordinate care with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building per the Treatment Plan.
• Attend and actively participate in weekly Treatment Team meetings.
• Link clients and families with services and resources identified in the treatment plan.
• Act as liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support for clients as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
