Job summary

Housing Works seeks a Harm Reduction Specialist in New York to serve as a bridge between clients, staff and medical providers. You will conduct assessments, enroll clients in case management, provide health education and coordinate services to promote recovery and wellbeing.

You will maintain documentation, support group activities, and assist in navigating insurance and community resources while engaging clients with culturally sensitive outreach and timely communication.

Qualifications

• Minimum of a high school diploma or General Equivalency Diploma required.
• Experience providing care navigation services preferred.
• Bilingual in Spanish/English helpful.

Responsibilities

• Conduct initial assessments to evaluate health and wellness status and care access.
• Provide health education and risk-reduction interventions.
• Maintain care coordination with primary care and behavioral health services.
