Job summary

Housing Works Inc. is seeking a Case Manager in New York, NY. The role involves intake interviews, care plan development, care coordination, and connecting clients to health services and social supports.

You will work within a harm-reduction framework to support individuals with substance use and chronic health conditions. The position runs Monday through Friday, 9 AM to 5 PM, with opportunities to contribute to education and outreach efforts.

Qualifications

• Strong data entry and documentation skills.
• Excellent communication with clients and teammates.
• Experience with case management and care coordination for marginalized populations.

Responsibilities

• Complete initial assessments and develop individualized treatment goals.
• Coordinate care plans, referrals, and discharge planning.
• Provide harm reduction education and Narcan dispensation as needed.
• Maintain client records in EMR and ensure timely documentation.
