Job summary

The incumbent will work on risk assessments, education, and collaboration with a multidisciplinary team to deliver person-centered care and effective outreach across Manhattan

Qualifications

• High school diploma or equivalent required.
• CASAC-T or enrollment in CASAC program within six months of hire, with completion within eighteen months.
• Minimum of two years of experience providing harm reduction and substance use services.
• Experience with homelessness, mental illness, substance use, HIV/AIDS, or chronic medical conditions.
• Ability to work independently and as part of an interdisciplinary team.
• Strong organizational skills and ability to manage a diverse caseload.
• Ability to work under pressure, including in crisis situations.
• Experience conducting risk assessments and making treatment referrals.
• Experience with community outreach.

Responsibilities

• Engage individually with tenants to support harm reduction goals and housing stability.
• Develop and facilitate workshops on recovery education, healthcare self-advocacy, coping skills, and social support.
• Collaborate on-site with leadership on community outreach and education tables.
• Document services and assessments in line with program requirements and funder standards.
• Participate in staff and clinical meetings to ensure best practices and coordinated care.
• Engage in training and pilots to enhance service delivery.
